Central Ohio Behavioral Medicine, Inc. (“Company,” “COBM”, “we” and “us”) takes your privacy seriously.   We have developed this privacy policy to let you know how we collect and use the personal information you provide to us while visiting the COBM web site (the “Site”).

Information We Collect

We collect personally identifying information (name, email address, mailing address, industry, occupation) which you provide to us when you register as a member on the Site (“Member”).  The Site also collects which sponsor or advertiser (collectively “Sponsors”) information you view on the Site “whether or not the Sponsor’s bar code is on your art, what parts of our Site you have visited, what features you use most.  This information is associated with you personally if you are a Member or if you have otherwise volunteered personally identifying information about yourself.  We also collect such information from other users of our Site who have not provided personally identifying information.

How We Use Cookies

If your browser is set to allow cookies, we may use it to store small amounts of information on your computer about the subjects described in the previous paragraph.  Cookies may be used to limit the number of advertisements you see while on our Site, or choose what advertising may be of interest to you.  It is not necessary to have cookies turned on to visit our Site, but without them, your access to some areas may be limited and certain features may not be available to you.

Use of Personal Information Collected
The personally identifying information you provide to us will be added to our databases and used for future communications regarding site updates, upcoming events and new services. Unless you provide an “opt-out” notice in accordance with the instructions on the Site and in our communications, we may provide your personally identifiable information as part of our visitor and member lists to Sponsors, and may provide it to relevant third parties if we believe our Members might be interested in receiving an e-mail or other written correspondence from any such third party.

We may also share with our Sponsors your personally identifiable information, as well as non-personally identifiable information that we collect, to help Sponsors better understand our audience and to evaluate the reach and appeal of their advertising and information on the Site.

Emails from Company

All e-mails sent by Company are compliant with the CAN-SPAM Act of 2003.  All e-mails contain a valid opt-out mechanism, the valid postal address of Company, an operative return address, and an accurate and explanatory subject line.  At any time, subscribers may opt out of our e-mail list by following the opt out instructions in their current e-mail.

Your Right to Opt-Out

If you do not wish for your personally identifying information to be used by Sponsors or other third parties, you may opt out of this service by mailing us a letter at the address below, specifying that you have elected to opt out of any third party mailings.  Include your full name, address, and e-mail address to:  Central Ohio Behavioral Medicine, Inc., Privacy Manager, 5025 Arlington Center Boulevard, Columbus, Ohio 43220.

Childrens Privacy

Minors under the age of 18 are prohibited from using the Site or being Members or Sponsors.  We do not intentionally collect or maintain information through the Site from those we actually know are under the age of 13, and no part of our web site is designed to target or attract anyone under the age of 13.  No one under the age of 13 should provide any personal information or participate in any forums, chats, or online discussions.

Account Information

During the registration process, new Members and Sponsors are asked to disclose personally identifiable information.  This information is stored and used to provide services to our Members and Sponsors through the Site.

Terms of Use and Policy Changes

Company reserves the right to access and disclose personally identifiable information to comply with applicable laws, lawful government requests and court orders.  Company may amend our privacy policy at any time by posting a revised version on our website at http://www.mycobm.com Information collected before changes are made will be secured according to the Policy in place at the time such information is collected notwithstanding future changes to this Policy. The practices and policies in this Policy replace all previous notices or statements with respect to the same subject matter.  This version will be effective at the time we post it.  We have not previously posted a privacy policy for the Site.  This privacy policy is effective on January 1, 2013.

Your California Privacy Rights

California Civil Code Section 1798.83 permits our Members who are California residents to request and obtain from us once a year, free of charge, information about the personal information (if any) we disclosed to third parties for direct marketing purposes in the preceding calendar year. If applicable, this information would include a list of the categories of personal information that were shared and the names and addresses of all third parties with which we shared information in the immediately preceding calendar year.  If you are a California resident and would like to make such a request, please submit your request in writing to the address shown below.
